Fee Waiver Application

Instructional Computing Fee waivers must meet the following criteria which have been set forth as policy by the Institute of Technology Instructional Computing Committee:

  1. You are not currently enrolled in a class requiring the fee.
  2. You need the account because you are an IT Labs consultant, an instructor or TA, or a student making up an incomplete. You must also meet qualification #1.
  3. You must have a professor or other appropriate person sign this form.

You must take this completed form to EECS room 1-213.

Institute of Technology Systems Staff reserves the right to contact the
professor (or other appropriate person) for verification
and to deny the waiver whenever appropriate.

Please print legibly; if we can't read it, we can't waive the fee.
